There is a lot of consanguinity on the island, similar to the State of Alabama, so there's that.

Check out the phone book, there are only one or two family names.

I have been to Grand Cayman a few times and it is always very nice. What's really nice is that they speak English and you can drink the water. The food is really good as well.

Same on most carribean islands outside of Jamaica/PR/Haiti. Its due to the plantations (mostly sugar) that were once there. Once slaves were freed they took the last name of the plantation. On most of these islands you only had room to suit 2-3 plantations. Hence only 2-3 last names for most of the residents.
