However, insecurity is a completely internal state, and is often fluid. Narcissism is rigid (personality) has that, but has some observable behaviors as well. 

I'll be honest, Trump could very well have a narcisstic personality disorder. Narcissism is actually a common trait with entrepreneurs, and in fact, it may be a positive in regards to success. It's also a common trait in politicians as well. 

But like anything else, narcissism can be channeled into a positive (motivation to be successful), with the expected drawbacks.
