Yes the dentist did mine. Will mix up a powder and make a pudding looking substance, and coat a mouth piece that you will bite your upper teeth onto.
It is really uncomfortable and hard to get into your mouth, if you can't open too wide like myself... once in, it takes maybe two minutes if that to dry. They will either send it off or do it in house, and take a hard mold out of cement or clay or something IDK what it is... from that they will mold a soft plastic, which is your mouth guard.
After I brush my teeth every night and head to bed, I pop it in and it helps out. The first night or few, you will feel weird having it in, just not being use to it, might drool a little and just feel like its uncomfortable, but you get use to it and I actually enjoy it now. After sleeping with the soft one for a few months, if you feel you still have an issue and bite down hard on it at all at night and want to move up, they make a harder one, once you have gotten use to the soft one.
