That is only an insult only if it attributes some form universally undesirable physical attribute onto one's character or morals that can be universally or at least contextually understood within your intended target audience.

Toe spreader just makes me think of sitting on the beach with a beer in a low-profile beach chair listening to Blind Melon spreading my toes in the sand.
