Trim down on the ridiculous beaurocracy to start. Hire better practitioners and pay them competitively while also offering some of the best benefits in the business.
And finally - this is important - actually FIRE bad staff.

The VA is a broken system. There is no fixing it. To many untouchable bureaucrats running it for any real change to happen.
Like someone said. Shout it down and give Vets good insurance plans and let them choose where they go.
