OT docs. Treatment options for sore throat/fever

Went skiing 2 days ago and probably swallowed some water. Throat is red and has white patches. Also running fever and getting chills(haven't taken temp)

Should I take medicine and ride it out or go get a shot from the doc-in-a-box

sounds to me like you have a pretty bad infection...I would not consider it an emergency, but some antibiotics would get you feeling better pretty quickly...

Sounds like you need to see a doctor or urgent care. Could be strep. Which you need antibiotics if it is. They can do a rapid swab there and tell you if it is.

Yeah probably strep

Ask for a steroid shot and you will get some omoxicillan.

You will feel 10x better in 24 hours

Strep will go away with or without antibiotics, they are not necessary. It may shorten your illness briefly and make you feel a little better but there is no risk ofrheumatic fever anymore which is why they were classically prescribed.

It's still a 1A recommendation to treat Streptococcal pharyngitits with antibiotics.


ETA: Specifically confirmed GAS. It's 2B to treat on suspicion for GAS.

No I meant rheumatic fever.... Scarlet fever is not a harmful complication, it's a symptom

I still test for strep but only give abx for confirmed + cases, which is what the IDSA recommends.... I'm just saying evidence supports it will get better no matter what you do with extremely low chance of any consequences

Fair. But AAFP suggests a Centor score of 4 or higher correlates with a 51-53% chance of a streptococcal origin for pharyngitis. Treating under that suspicion before the Rapid and culture come back is basically your own clinical style. Neither way is right or wrong.

Yeah but the rapid strep is more sensitive and specific than centor criteria

I thought you had to treat strep or the streptococcus could end up harming your heart?
