Physical Therapy is pretty good but I have found PT's seem pretty conservative and their plans are tailored to a pretty low level of function / outcome. I had one tell me once I needed a medical discharge and that I would never function again. I went on to do some pretty decent things when I told him to frick off.
The military has done quite the number of my joints. They've told me through the years "just rest, that's all you should do". One week of rest and I feel like I'm 80. One week of hard exercise and I feel like I'm 20.
Consult with regular doc, or ortho, and PT - but look into beefing up your exercise routine and if you have the cash, consider getting a Reverse Hyper machine. You can search for them 'westside barbell reverse hyper'. Of course, if able, you can try to manufacture an equivalent yourself as a poor man's substitute. Something is better than nothing.
As I said, talk with your medical peeps but I have never personally encountered someone with back pain that didn't benefit in a major way from really stepping up the exercise stuff, especially foundational exercises, and people typically rave about that reverse hyper.

This is beyond true. Resting is what triggers my back pain. It seems counterintuitive to exercise when your back feels like it’s being stabbed, but it’s truly the best thing I can do. Looking into the McKenzie & reverse hyper suggestions you & another poster had.

I've been having severe pain in my mid back area moving around to my side. It's mainly after I get home from work. It hurts like hell.

Tried that for my neck a few times with no help. Had to have C5/C6 fused together. That has been 12+ years ago. It now is acting up again due to DDD above and below.

Agree with you for sure. If it’s a PT giving you basic mat exercises and “core strengthening” and you’re already in decent shape, that therapist doesn’t know what they’re doing or is lazy, find another PT. Quality control can be a real issue in our profession.

Weird that it hit me at age 28 too. Sciatica , and losing the ability to run comfortably were the worst part. Now , more than two decades later it is mostly general stiffness and lack of flexibility.
If there is one piece of advice I could give you it is to keep moving . I would also limit the high impact stuff as much as possible. Unfortunately that will make you feel old. I went from daily trail running to no running. It’s a mental adjustment more than anything. You have to keep your back strong for age 48 and age 68. So stay very active but limit high impact stuff.
Physical therapy never worked for me . Stretching, soaking and movement throughout the day . I walked like 1800 miles last year if that gives you an idea.if I walk- no sciatica.

If you’re looking for a neurologist in Baton Rouge, mine is Dr. Brian Murphy at he Br clinic. I actually have an appointment in 45 minutes to go over my MRI results. I have thoroughly enjoyed him and truly believe I am in good hands with him.

Walking is the only thing that has truly started to make me feel better, I try to walk a couple miles every day now. I drove to north LA this weekend and when returning home after the 5 hour drive my whole back was spasiming damn it hurt. I immediately went for a long walk and felt much better.

Sitting puts your spine in a flexed position. Get a lumbar roll.

Was diagnosed in my mid 20s.
Basically means you have an arthritic back.......some have it worse than others

My life has centered around walking since the late 90s. Vacations, rain, incredible heat- I’m walking. Unfortunately my stiffness/ flexibility is really bad - but I refuse to take meloxicam and all that other stuff. A lot of the people you see walking neighborhoods in their 40s / 50s every day are doing it to ward off sciatic nerve pain. Podcasts and Spotify have made it so much more productive. Will walk six + miles today .

Try 1500 mg of curcumin to reduce inflammation. My lower back pain and leg symptoms have been reduced tremendously in the last 4 to 5 weeks since I started doing this.

Yep, back in '85 when I was 32, doctor diagnosed me and said it was something that probably started at birth. Now I know why those hardass basketball courts were causing me a little pain.
Anyway, the doc said I had two options - no operation but maintain the mobility I had and endure what pain came with it or have the operation, relieve the pain (supposedly) but be less mobile than I was. I told him "hell, I've learned to live with the pain and if I'm going to be less mobile than I am now, leave the knife in the drawer". He told me eventually that I would have to have the operation.
33 years later I'm still mobile, little pain and still no scar across my back.

Just curious when some of you were diagnosed with DDD was your disc bulging or herniated any at that time? or did it just show a generally drying out? My L5-S1 report read L5-S1 degenerative disc disease with mild dessication and small broad based central disc protrusion, and foraminal stenosis.

Wouldn't go this route unless you can't walk or the pain is so great you want to suck start a shotgun. Recovery is a bitch and its 50/50 on being a success.

Diagnosed with it in my neck, over 20 years ago. Doc said I would know when something needs to be done about it. Boy, was he right!!! Surgery was necessary, but it was like a walk in the park.

Went to the doctor this morning. He did x rays & referred me to PT. I’m waiting for the x ray results.
I hate going to the doctor. I always feel like a hypochondriac.
