I don't the specifics of the case. But if you have a willing donor that matches the recipient that's who'll get it. If the willing donor doesn't match, they can enter a pool with other willing donors and can essentially "swap" if appropriate matches are made. Win-win.
As far as I'm aware theres a separate process when it comes to deceased organ donation.
That's my cursory knowledge of how I think it works
