For me it's the scene in Zodiac where the guy is bound and stabbed in the back repeatedly while with his girlfriend at the park. It just disturbs the shite out of me.
My wife says for her it's the final scene in Looking For Mr. Goodbar where Dianne Keaton is stabbed to death while having sex.

I should caveat this by advising that I do not watch and do not care for much in the way of the slasher horror genre. I can appreciate the classics like the old school Bela Lugosi and Boris Karloff stuff, as well as films like Psycho and Halloween (1978).
However, in a film that is otherwise a high quality product, I would say that Se7en contains the most disturbing scenes - for me - at least as far as what you're asking for in this thread.
On the other hand, I find certain scenes in films based on actual events even more disturbing - Blackhawk Down (death of CPL Jamie Smith), Schindler's List (self-explanatory), being notable examples.

Running Scared: the scene where Paul Walker's wife goes to the crazy pedophiles house and realizes that the parents are pedophiles and have kidnapped her son and his friend. The pedophiles dress the kids up then cut them up and put them underneath the floor mats. I was about 14 when I first saw this and it scared the shite out of me.

The scene in Mission To Mars where Tim Robbins takes his helmet off in space. I was just a child when I saw that and it gave me nightmares for days

1. The two naked people getting pummeled by rotten meat in the back of a truck in The Cook, the Thief, his Wife, and her Lover. Disgusting.
2. The Mist - Dude killing everyone in the car (including . . .), then stepping out of the car and realizing he just needed to wait a minute longer. Horrible.
3. Everything in the original Texas Chainsaw. And it is almost a bloodless movie - everything is implied. Yuck.
