Last summer I was having trouble getting my blood pressure regulated, I had lost a lot of weight and as a result it was running too low, 95/60 was common. One night I fell asleep in my recliner, my wife woke me up to go to bed (I'll skip on that logic), I took a few steps toward the kitchen and I just went down hard, hitting my neck on the granite countertop.
EMS came, put a hard collar on me and took me to ER. Over the next few hours I found out that I had fractured C3-C5 vertebrae. I was in severe pain, but not paralyzed, neurosurgeon came in, he told me he would operate on me the next morning putting the pieces back together with screws.
The next morning he came to my room a couple of hours later than I had expected him. He told me he had spent a lot of time going over my CAT scans and MRI, he found that none of the bone fragments were near nerves that might paralyze me, so instead of risking surgery, he wanted to keep me in a hard collar for 90 days, then do another CAT scan.
While 90 days in a hard collar was pure torture, it was the best alternative. He also told me that be on the lookout for collateral damage, for someone my age (72) to take a fall like that, usually other things get hurt too. Over the next few day I found out that I had aFib and some of the bone fragments were lodged against my esophagus, making it difficult to swallow.
I was in the hospital for a week mostly due to the pain, I refuse narcotics, but when I could at least sit up on my own, they released me. I was not allowed to lie down, so I had to sleep in my friend, the recliner.
Recovery: It's been a year, I will never have much mobility in my neck, I can't look up, left about 20 degrees, right about 5 degrees, there is no treatment that can make that better. I was able to drive again in January, but with my neck I plan my routes carefully to avoid looking right. I'm having a cardio ablation next week to get rid of aFib, the swallowing issue cleared up on its own.
There still is pain, but its not severe, I put it in the "others are much worse off" category. After 90 days I got out of the hard collar and 30 days in a foam one. I consider myself to be fortunate to not have any paralysis, I did a lot of PT, not sure if it helped a lot. The mental side it very important, I work out 3x a week and keep working so I don't fixate on the pain, everyone my age (73) has pain.
Good luck to you and your brother, having supportive family is a huge boost, try not to worry about his walking, what a person can do after a serious accident is a lot more important than what he can't do. It must be very difficult for you to see him in ICU on a ventilator.
Prayers to your whole family.

My Aunt did, lived another 10 years or so after with limited movement but she could still function.

I broke C4-5 and had to have a cervical fusion, got staph infection in the wound and had to have multiple surgeries. My legs weren’t affected, but I had to learn how to write my name again and had pain/numbness in my hands that lasted about 3 years after the incident. It took about 5 or 6 years to feel completely normal again and I just ran my first marathon a few weeks ago

That is awesome and good to hear of recovery stories. My brother was a runner but gave it up the last few years. Hopefully one day he will be able to go at it again.

Same exact injury. The staph infection was awful. I was home recovering from surgery and started to feel like I had the flu. Back for another surgery and week in the hospital. Had to have a picc line put in for a while afterward.

Well there are different degrees of broken necks. Back in my accident in 99 I had a broken neck. But my “broken neck” was just a fracture in a vertebrae. Whatever scale there is for broken necks, mine was the absolute lowest I guess. I never spent a minute in any kind of traction.

My gf (no pics) tripped over storm debris following Ida and fractured C3-5. They used rods to fuse them, and today she just deals with the stiffness and challenges getting comfortable to sleep. Her biggest regret is not being able to look back to back down the boat trailer (love Louisiana women, they’ve got priorities )

Back in March 2020 I had an accident and broke C5 and C6. Had surgery and they fused C5-C7. Took a bone graft from my hip for the fusion. Never lost any mobility in my extremities. I do occasionally get a numbness in my hands depending on my posture. Wore a hard brace for a few weeks. Have most of my left and right turning. Lost more motion looking up. It was interesting for the first few weeks trying to finish any drink. I could barely raise my head high enough to finish a soda, so it looked like I was trying to do the limbo to get the last bit of drink from any cup

That injury looks to be at C6-7 so he will be able to breathe independently since the diaphragm is fully innervated. It would be positive if he can move his fingers.

He does move his fingers but it is limited. Had most movement in his arms and strangely his neck.

Buddy of mine broke his in 2010. Your brothers image looks eerily similar, except my friend also had a fractured skull. I thought he was going to die. 3 years later he started running, year 4 he ran his first marathon. He does have limited upward movement, but other than that he doesn't have any issues. You'd be surprised what the human body can do.

He is still in icu trauma at university. He may go to touro for rehab. We still don't know where or when. I'm guessing he still isn't out of the woods.
