Over the past ten years, doctors were pressured into "pain is whatever the patient says it is, the fifth vital sign, and you have to treat it" around ten years ago. Now with the heroin epidemic, everyone wants to blame the doctors for prescribing opioids. Can we ever get a happy medium again?

Everyone's pain level is different.

When I broke my back, I said it was a 5-6 even though it hurt to breathe.

Getting stabbed in the forearm was a solid 8.

Touch my foot when it's asleep and I'll murder you. That's a 10.

I fractured a rib about 7 weeks ago. My pain level was around a 7.. Bruised lung, Fractured rib, major bruising on my leg and arm.. they gave me a small dose of morphine (after I suffered through x-rays) that wore off well before I left.. I was there from about 8:30pm until 3am. That shite sucked.. but I got a weeks worth of Percocet, at least

Anyway, are they cracking down on the dispensing of painkillers or did I just receive bad treatment?

I'd say 75% of new patients I see describe their pain as 10/10. People are retarded and don't understand how scales work.

When I'm treating patients in the hospital I always ask them their pain on the 1-10 scale. I explain in detail how a 10/10 is essentialky excruciating pain that will cause a pretty significant response and cause them to be very uncomfortable.

That's usually when they calmly tell me their pain is "oh about a 9 " while reclining in their hospital bed and watching the price is right.

Last time I was in the ER post accident I had three or four fractured vertebrae. I was standing there at the nurses station. Some Mexican dude was rolling on the damned floor. I asked the nurses if he needed help.

Their response, he does that whenever he runs out of meds.

I legitimately could have killed him and felt nothing.

I am not sure I believe that the opiate epidemic is a result of doctors prescribing meds to people who are in pain.

frankly, why should people who have legit health issues have to be in pain in order to keep someone who just likes the buzz from getting himself in trouble?...

Part of the issue is the scale of the person.. if they've not had any major injuries before, then their scale is going to be entirely different than someone who has had major back injuries or something like that.

When my wife sees them in outpatient and they say 10/10 she tells them she's going to call 911

I personally enjoy telling people that the scale only goes to 10 when they give a 15 for their pain when not in visible distress

Yep, you can ignore that foolishness. An infected ingrown toenail is about a 7. An infected ingrown toenail that someone just dropped an unabridged dictionary on is an 8. Add to that a perfectly timed kick in the balls and I figure that's a 9.

I always explain it as 10/10 being "emergency room pain". That doesn't seem to dissuade them from telling me that this particular issue which they have waited months to seek treatment for, and with which they walked under their own power into my office with is causing pain that literally could not be any worse.

What is so retarded is that we're obligated to document whatever they say. I can't convert it to normal, sane person scale once they leave.

Are you obligated to treat the pain as a 10? If I was a junky I'd be scared to say 10 because they'd think I was just looking for pills.
