The only time I ever wedged something under the handle and went inside, I ended up pumping roughly 11 gallons of fuel into the parking lot.

The nozzle will stop the flow as long as it is in working condition. They do fail and when they do you get an overfill.
Then people want to argue about who pays for the spilled fuel and/or it can (and has) start a fire. Overfills make a special mess you'll notice at the diesel islands.
For years the State Fire Marshall's office in BR required the locks to be removed as part of the final inspection. Then they stopped enforcing it and it has become a choice of the station operator. In particular, Exxon required them to be removed (but now all Exxon in BR are owned by Circle K so you may find them now at some locations).

I thought the removal of these things was an EPA thing. Have seen a few with a "remove by" date on them

I overfilled the other week at a hands free pump somehow. Couple quarts of diesel down the side of a brand new Passat. I was real pleased.

Two of the stations I've been to in BR have hands free on some pumps at the station, and some pumps where they have been removed. You just to have to pull up to pump at the right island.

Press the handicapped assistance button on the pump. Tell them you have arthritis. Every time. Watch them pump it. File a complaint with their distributor if they complain.
