Honorable mention for John Kramer (jigsaw) from the original Saw.

Darth Vader.. no contest.
I can't disagree with this strongly enough. In fact, I'll even go so far as saying tht I don't think I could be friends in real life with someone who takes the above position, and it might even be a relationship gamebreaker unless the girl is just like a legit 9 or 10 and otherwise cool.
But if she said that the greatest villain was darth vader and that her favorite movie is pretty woman and her favorite tv show is Friends, it wouldn't matter if she looked like prime Anna Kournikova. Just wouldn't work.

The chic from the movie Hard Candy, although I'm not sure she's a villain.

DV, Ledger's Joker, characters like that don't really come into it for me. They're great villainous characters, and may fit what other people are looking for when it comes to "best", but they don't make me seethe with hatred, and that's my criteria.
Darth Vader is #1. He is probably also the most well known and recognizable movie character of all time. People who have never seen a single minute of any Star Wars movie could identify DV.

He what Dr. Doom should have been modeled after.

I think as a whole though its hard to top Javier Bardem's performance in No Country For Old Men.
Entirely in a class of its own with the haircut, the boots, the cattle brain prod, the silences shotgun, the coin.
I'd debate that one until the cows come home. He's hands down #1 for me behind Darth, just because he is untouchable.

I hated this son of a bitch.
But yeah, Vader is the correct answer.

Maybe not the best but definitely great, Mads Mikkelsen as Le Chiffre in Casino Royale.
