I would probably go dressed dressy business casual (i.e. button down shirt or sportcoat at max).
probably would just order something simple to munch on with a cup of coffee (i.e. an English muffin, etc.)
be ready to tell him about yourself and your experiences, as he is trying to see whether you will fit in with he and the others that work there....be friendly, but don't overdo it.
he will likely be basically steering discussion to see how you would handle certain situations, so have a few very short stories to tell...that is, be able to tell him about a time when you dealt with a difficult individual and handled it very well, a time when you had to compromise and it worked out well, etc.

Whatever you do, make sure to poop before you go. Wearing a suit in the summertime + having to poop is a recipe for you looking like a sweaty cokehead, and sweaty cokeheads rarely get jobs.

You don't have to show him your resume if he doesn't ask for it. But it's better to have it available, even if you end up not needing it.

I do this before every business dinner that I go to. I also never eat salads. Way to messy. If I have an appetizer I get soup.

It's not in NYC. It's summer. Don't wear a suit. Wear a jacket, lose the tie. He won't even remember what you wore. Other than that, be prepared with good, genuine questions that you come up with yourself. You might not even use these questions, but in case of awkward silence. Just have a conversation.

Over prepared > under prepared.
Also, look up the company of the place you're interviewing with. Know about them, have some questions about them specifically too. The managing director would be flattered you know about them and also when he asks you, "Why do you want to work here rather than any other investment banking firm?" You don't have to make up some bs answer on the fly.

Nope, nope, nope. This is how you properly prepare for interviews and I speak from personal experience within this industry.
You can be cool, be smart, AND be prepared at the same time. Try to be Mr. Cool by not wearing a suit and watch him show up in a suit (now you feel underdressed) and then during the interview he says "you're going to want to write this down" -and you just sit there awkwardly because you have nothing to write with.
If wearing a suit makes you so uncomfortable that you can't function then yeah maybe don't wear a suit, otherwise wear a suit. It's not that difficult and it will literally make no one feel uncomfortable if you're the only one wearing a suit... Have some confidence

I guess that could work, but so much for shooting the bull and cutting up a little if you go that route and start recording. It's 2016 so you never know, someone will want to sue you for something... just think this could make the interviewer uncomfortable, however I don't think a tie and a notepad would. If this is investment banking we're talking about this is the norm.

By the sound of this thread I would suck now at interviews. But I would say.. Be yourself man. Tell them that too. "Look I am here because I think I'm the right person for this job. However im just not a formal person. I'm going to order the unlimited pancakes because it's a no brainer. Much like the co workers you've come to know for the last 10 years at the workplace, if you brought them here this is what they are getting too. I mean..its unlimited pancakes! I have a passion for things outside of work and life is too short to be a robot today. I can tell you that throughout your questions you'll find that I am 100% capable to do this job but what it may not be able to tell you is that I'll make a lot of friends at work, I'll work later than my shift requires and I may get annoyed when someone wants to talk about football because I'm focused on something more important. I'll have people I don't even know yet vouching for me when I go for a promotion in 2 years. I may not be a picture perfect interview but when you make your final decision your going to know the real me so either way, yes or no, that lets me sleep at night knowing I tried my best for this position"

I will say this not from experience but from some radio program that I heard a while back. They were talking to some big shot CEO that everyone would know if I could remember his name...but I can't.
Anyway, he was saying that he always interviewed over a meal and would have the restaurant intentionally screw up the interviewee's order, be rude, and just all around bad service.
His purpose in doing so was to see the interviewee's reaction. You may want to consider that he may be gauging your interaction with people when things are challenging.
