Besides the crappy service at locations too numerous to list, the worst was when I was served raw grilled chicken.

I understand the need for that handle now, Nap. I doubt that behavior was an isolated event.
I really don't have any awful experiences that are unusual other than the typical waits on food, tables or food that wasn't up to par. Best I can think of is ordering "New Orleans Style BBQ Shrimp" at a restaurant in FL years ago before I knew better. I received a bowl of flavorless boiled shrimp (I could tell by looking at them-I didn't taste one) with a bottled bbq sauce poured over them. Returned them to their maker with advice that this was not what was represented on the menu and had them removed from my bill. Ordered a nondescript burger instead.

Au contraire Gris. Normally I am the picture of sophistication, class, and composure. It was the booze that got me. My grandfather was 1/4 Cherokee.

Wife and I were completely ignored at the bar. Finally got seated and the waiter was more interested in another table where an another employee came to eat on their night off.
We went for their lobster night and after seating us they informed us they ran out of lobster yesterday.

Burger king in the Aquarium (about ten years ago).. a fricking mouse ran under our table.

Went into the Hilton Hotel restaurant in Nashville, Tenn back in the early 80s. It was 17 below zero in the worst freeze in the history of Nashville.
Waitress: Hi what can I get for you?
Me: I'd like the fried shrimp platter.
Waitress: I'm sorry, we're out of shrimp.
Me: OK, then I'll have the Grilled Chicken Caesar Salad.
Waitress: We don't have any. Actually, sir, our trucks cannot get through in this ice.
Me: So, what do you have?
Waitress: I've got 2 little boxes of Frosted Flakes leftover from breakfast. But we don't have any milk.
Me: I'll take them.
Waitress: Hi what can I get for you?

It is a funny anecdote to watch the shelves eympty of certain items when the words ice or snow are mentioned in the forecast in the south.

Nothing crazy but a few months back I went to Romacelli in BR. We were seated outside on the patio pretty quickly, so that wasn't bad. It was pretty clear all the people around us were just friends of the employees who didn't need much attention. We didn't see our waiter, or any other staff member for that matter, for 15 minutes. They took our drink order (with 0 knowledge of anything on the menu, food or drink) and disappeared for another 15 minutes inside. When our waiter finally out to our food order, his shirt untucked, we had to repeat everything we said 3 or 4 times.
Whilst I was trying to put in my order, another employee decided he was ready to go home. Since the patio is pretty small and our waiter was blocking the walk way, the employee keeps saying "hey mike..!... mike!... hey mike, can i get through?... hey mike, watch out." So, Mike finally steps aside and then, said Employee slings his bike over his head and proceeds to bump into us as he's trying to maneuver his way out as his muddy tires are dangling over our drinks and "clean" table. I was looking around trying to decide if it was a joke or something... it wasn't.

My review of Village Grille in Shreveport:
For my wife's birthday, I treated her to what I thought was one of the best steaks in Shreveport. My wife is a huge fan of red meat and had been clamoring to try the Village Grille.
I made reservations earlier in the week and was told we would have a nice little table set aside since we were celebrating. We arrived at dinner a few minutes early and were seated at a booth in the bar. We thought it was a little odd since they had promised us a special table but we tried not to let it bother us.
The waiter immediately came up to introduce himself. Without so much a offering us a wine list or inquiring about something to drink, he removed our wine glasses and walked off. He returned with waters and jumped right into the appetizers, never asking us if we wanted something else to drink. After I requested a wine list and asked for suggestions on the Cabernets they were offering by the glass, the waiter pointed out one cab he liked, but quickly dismissed it saying "...but that is probably too expensive for you." That is absurd. I should never be judged by a waiter. My wife and I look young, granted, but were dressed nicer than almost anyone in there (bow tie and jacket for me, dress for her). This is the kind of bush league service the Village shouldn't tolerate.
My wife got the Kobe steak and I got the lamb. Now, I've never had Kobe beef before, but I had high hopes and we were both disappointed. The $65 Kobe steak was pedestrian at best. It was not particularly tender and while tasty, was nothing to write home about. The lamb I got was alright, but again, not worth the $40 price tag.
After our waiter served our entree, he didn't stop by again until he brought us the check. I had to flag down another server to order our second round of drinks. We could see him the whole time servicing a large table in one of the main dining areas and just flat ignoring us.
All in all, we were very disappointed in our experience. I would rate it a 3/10. I don't mind paying for a nice dinner but a $250 ticket for terrible service, a booth by the bar, and average food is disappointing. I hate being judged by the fact that I look young and I really despise being dismissed by servers. This is one restaurant I will avidly advertise against to anyone who asks.

At Poor Boy Lloyd's in BR in 2006. My brother ordered a steak rare. They bring it out medium well. He eats a couple pieces and realizes the mistake. He tells our waitress about it and she kindly says they will cook him a new steak rare. He isn't hungry any more and says to just take the steak off the bill. The waitress says ok. The owner or manager comes out a few minutes later to ask what the problem was and that he wants to cook my bro a new steak. My bro repeats that he does not want a new steak. The owner or manager takes his plate with the well done steak and says loudly "Are you sure!?!?" Then walks over to the trash can and says loudly "Are you going to make me waste this steak?! It's just going to get thrown in the trash!!!" We just kind of didn't respond and be made a scene by jamming the plate into the trash can while staring us down. We were all just so perplexed by the strange behavior.

Personally I don't have a problem with a waiter dumping red wine on the head of a Bama fan, good show.
One of the best and one of the worst, was at some hot new place in Metairie in the 80's, off Cuaseway, I don't remember the name, I think it had a huge aquarium. Anyway, my wife and I were taking the CEO of my company out to dinner and someone had recommended this place. We had a fine meal then we ordered after dinner drinks and dessert, and the service died. Could not find our waiter, I got up, found the manager, asked him to to please send our waiter, still no one. My boss was fuming, by this point we just wanted to pay the bill and leave.
Finally, about half and hour after we lost the waiter we just got up and left. It was likely at least a $150 bill, but I never felt guilty for a second.
