Just FYI, local a esthetics don't work we'll at all on infected tissue. That's the most likely reason for you hurt like hell.

He's probably talking about an intrapulpal injection. Sometimes a "hot" tooth won't get numb from a local injection or a nerve block. In these cases, sometimes an injection directly into the pulp (or "nerve" within the tooth) is the only thing that will get it numb. But first you have to drill through the enamel and the dentin, which will hurt like a sonofabitch since it isn't numb. Once you drill "deep enough" and expose the pulp chamber, you can then inject directly into it and finally get adequate anesthesia.
And a good dentist can handle just about any routine root canal, but a good dentist will make even the most difficult root canals look textbook. I almost never did molars, unless things were really slow at the time. Occasionally I'd throw the endodontist an easy tooth, but only if it was a real pain in the arse patient.
ETA: despite root canals being one of my least favorite things, my success rate was well over 95%. The key to a high success rate is good case selection. But I've seen endodontists do things that I considered physically impossible.

I rarely did root canals on teeth that were hurting badly. I'd put them on antibiotics and good pain meds, it would settle down in a day or two, then I'd get them in when I could work when I had adquate time available. Typically that was a week later, but no one complained because they would virtually always calm down with antibiotics (even without swelling). If it didn't calm down in a day or two, I'd have them call me and I'd send them to an endodontist. Doing a root canal on a hot tooth that won't calm down is BAD for word of mouth and referrals.

Yeah, you will get some out of the blue that unexpectedly won't get numb for shite. And usually in the middle of those I'd be saying to myself, "Frick this, I'm never doing another root canal as long as I live."

I am in the Philippines now and the dental care is good and cheap. I can walk in off the street and get the work done. In the US I waited weeks for an appointment and had to go back several times and usually end up with a bill at or near at least a $1000.00. Every time I went in there was something that needed filling and it would cost me. Here I have never had a filling or any problems with my teeth in 10 years. I still have them.

I have met a number of people from Australia, U.S. and Europe who come here to get their dental done and the money they save pays for the vacation. The link is an example of prices in Manila, but I live in Cebu and the prices are even lower.

My real mistake was to get it filled by a dentist in Mexico. The filling sucked an allowed the tooth to decay to the point a root canal was needed. It was part of a 10 cavities for $300 deal. I got what I paid for.
