frick that gave me the chills... and i worked in an ortho trauma unit for almost 6yrs
that looks to me like a nail getting removed and having some serious digital blocking going on. No way you are pulling that barb out, so it would have to come out the other way..... removal of nail.

nah...I'd just take my dremmel tool and drill a hole in the nail...push hook through...a little neosporin and you'll be fine.

I have had something similar happen to me. They give you a shot to numb your finger that hurts as bad as any pain you could ever imagine. As soon as that is over you feel no pain. I had a sharp rock go into my finger and break off under my nailbed. Don't know how they deal with the barb, but when you are that numb they could rip off your nail and go forward to get it out. You won't feel anything for 6 hours
