Things that others drivers do that really irritate me..

Idiots who have zero depth perception when crossing a median lined street. They are usually 10' short of the traffic going in the direction they are trying to turn, and leaving their arse hanging out; blocking traffic going in the other direction.
People who come to a complete stop when turning right onto a side street. It's usually old ladies driving monster Cadillacs or Buicks.
..or other drivers in general. Nearly everyone else can't drive for shite.

They usually wait 15 seconds to turn, too, and you have to slow 30-40 mph to avoid hitting them.

People that slow down on the on ramp.
People that can't grasp the concept of merging.
People that drive slow in the left lane.
People that pull up to a 4 way stop before you and stare at you waiting for you to go.
TIMID drivers in general.

#1. Begin slowing down in the travel lane so they can get into a turn lane. Hey the whole road is a turn lane jacka$$! Get over and THEN slow down.
#2 is a variation that OP mentioned..leave a$$ of car sticking out so that no one else can go by while they sit half-in and half-out of the turn lane.

Driving in the left lane (highway) while not actively passing anyone.
Anyone in general going 10 under on a highway
Trying to merge from an on-ramp doing 20 below the speed limit (highway)
Driving up to the very end of a lane that's ending to merge when there is traffic (you're a part of the problem dumbarse)
Not other drivers but...Any left turn light at an intersection that only has green arrow or red arrow (no solid green light or flashing yellow arrow). I am perfectly capable of judging oncoming traffic and safely make a left turn.

tailgating (when I'm behind a slow poke or can't immediately get out of the way)
when I have proper headway between myself and the car in front of me and a dickhead hops in between us at 80+mph. The fricking headway is for fricking reaction time.
Also, the idiots in jacked up F250s that think their vehicle is now a sports car. It's not geniuses of physics......

-Pulls out in front of me and does not accelerate or tempt to match my speed to prevent me from using brakes.
-Pulls in front of me causing me to use my brakes while no one is behind me
-riding left lane doing the speed limit not passing or staying even with vehicle in the right lane
-going to merge on the interstate that has 70-85mph traffic and some drivers think it ok to merge in at 40
-Not getting over to further lane (if possible) to give extra room for motorist/officer on shoulder
-not yielding or recognizing when someone is towing a large load they can't stop quick and accelerating can take awhile.
I just drive with the mind set as OK I'm getting on such and such road that these drivers were on first. I try to do everything to not disrupt them (forcing them to use brakes/alter speeds)
Kind of like checking out at a store that has a line behind you. I have my card/cash/id already out ready to pay. If i have change and such i stuff it in my pocket and deal with putting in my wallet once in my truck. I just hate taking time/being in someones way for no reason.
