I've read that the USN deliberately avoids automating a lot of things that could be automated on subs, specifically to make sure there are always people in the loop who can respond to anything that might happen.

They, like the rest of the world, lack far behind the United States in naval capabilities. If you think the Russian military shouldn't be respected because of this you are mistaken. They have extremely capable missile technology, a very capable air force, and a very large and organized army. Maintaining a state of the art Navy is expensive and difficult and Russia has had great difficulty with this since the collapse of the USSR. Most nations don't even try.

as with most governments, you only hear about these things rarely because for everyone that is public knowledge there are potentially dozens that they were able to keep secret.

I wouldn’t say they’re cavalier about it, it’s not that they don’t care. They just view sacrifice as a social obligation.

An ET2 got a Navy Com pinned on him by the squadron’s commodore about 15 years ago for taking actions during a coolant leak caused by the shipyard. The CO wanted to mast him for “ violating immediate actions”. The Commodore said frick that and walked down and pinned him.
That says more about the training our nuclear operators receive prior to going to the boats, and the continual training at sea than anything else. The US is light years ahead of the Russians in that regard.

You know, I'm not usually one to call it, but I'm going to throw the bullshite flag on this one...
Total crew is around 170 with 16 officers and 16 chiefs. Manning slightly changeable, sure. FBM sub? Nobody calls them that. SSBNs or boomers, not FBM. But that's not the bs part.
You're talking about being in dry dock changing out a reactor, so a shipyard. They don't randomly run oxygen and acetylene lines. No explosive gas is run in lines anywhere because of the industrial environment of a shipyard. Even so, all of the temporary ventilation would've sucked all of the oxygen straight out of the boat. Maybe your boat was different, but highly unlikely.

Maybe it was a long arse time ago, like the 60s, and shite was different. He just said during the Cold War, which was ~45 years long.
