I could be off but I think it's 15 feet or higher requires a yo-yo.

Most places do. Also going down the scaffold ladders many places require the use of self retracting lifelines (yo-yo's)

Supposed to have yo-yos at 18' and above. I can't say anything about not tying off, I did it often. However, I always thought it was dumb that you had to be tied off sitting on a scaffold platform 10' above ground with a handrail all around you but you didn't have to be tied off going from the scaffold to the ground or vice/versa..... that's when you're more like to fall.

You don't have to according to OSHA as long as it is fully planked. That must be a GC rule.

It's my understanding is that it was the yo yo that failed but still not 100%.

I think this is right. Every contractor that I have worked for the rule was if your off the ground you have to tie off which exceeded OSHA standards but I do agree with his point somewhat. Most of the rules are meant to protect you even though some are whack and go overboard.

What do you mean by moving harness while going down scaffold.

Was he switching between yo-yo's at a deck or what?

This is always so surreal now that I'm in the business. We cover these topics everyday but something like this really puts it in perspective

Yeah I've learned a lot about this stuff in the last few years and it makes you really appreciate how dangerous this kind of work can be even with all of the safety measures in place.
Hell im and office guy and even had to take a pretty serious fall protection and scaffold construction over view course. Made me tie off and climb a structure too.

We recently received the findings of an event that happened with a scaffold death in south Texas.
Guy was doing his job and was tied off in what was assumed the proper way, but the scaffold structure collapsed into the water. With the weight of the scaffold material and his gear he got pulled under and drowned

I read on kplc's Facebook post in the comments section that he was climbing a ladder and there was no yoyo. Take that with a grain of salt.

Damn. When I was in equipment sales something like this happened at our other store in Mobile. Pipe fell off a truck and killed a guy, they rolled the phones to our branch and his wife kept calling but no one could tell her exactly what happened.

Can't imagine. Prayers to the family and his coworkers.
