Absolutely. My guys have been thrown out for looking like they were not tied off when they were.

I was welding a few yards away in the same cracker unit the next heater over when I heard a peculiar sound, one you don't hear everyday, and kinda paid it no mind. Then I heard an ambulance siren kinda chirp one time. Then my fitters told me to come off the scaffold, "everybody gotta go to the lunch tent". When I got down safely, using the yo-yo that I didn't feel really worked right, they told me a young welder had fell and was possibly dead! I was in total shock!! I work for the same company and I'm just so devastated this happened to anybody! My heart goes out to his family. It could have very well been me.

Site project management (FTI) had a 10 minute meeting this morning in all lunch tents across the site. Sub 10 minute meeting basically telling us to get back to work....

Doesn't make any sense to me, unless he was less than 20 ft from the ground. Otherwise, there should have been a yo-yo or ladder cage.

Should have been doesn't mean there was. I know at citgo they only recently introduced the ladder cage/yoyo rule and alot of the scaffolds built before that rule still havent been outfitted with them.

Many contractors require their employees to tie-off whenever they leave the ground, even on a complete scaffold. We don't require people to tag off on a complete green tagged scaffold but depending on height they may need to wear a harness to climb the ladder using the yo-yo.

Performance requires 100% tie off even on a green tag scaffold.

I used to work on that project. Co workers over there said Sasol/FTI had a 7 minute safety meeting this morning and told everyone to get back to work. EVEN the Cracker unit which was where the incident happened.

Pathetic.

Yea. Worked on the Aububon bridge and the new bridge in harrisonburg. Had one guy nearly fall out of a bucket tying an anchor cable but thankfully he was tied.

It was very disappointing. There was a moment of silence then senior management read the press release, followed by site fall protection requirements. Nothing else was said.

You could tell that they were forced to read a script, and directed not to comment any further.

It was very impersonal and cold given the circumstances.

Foolish to expect any more. The split instant something like this happens, the entire legal and HR arm of a big company goes into full litigation protection mode. Commenting beyond the exact words they were told to say would likely result in being fired.

I would have expected the project, or at a minimum, the unit would shut down for at least a day. That's pretty standard for investigation purposes.

Had a guy get killed in a tank field in the plant I was working in a few years ago. We had a moment of silence the next morning and they touched on what happened a little then it was business as usual. However, on the day of his funeral, they shut down ALL work in the plant and made everyone go to the offices or lunch tents for about 2 hours during the time the funeral was going on.
