It has changed over time. The vast majority of that has been dry humping; but in some movies lately, they have "sanitized" actual porn movies for it. Sometimes you can go on a porn site and see the hard-core version of the same scene.
There is so little soft-core now. The ease of access to porn ruined the genre.

Have you seen the movie Love Actually? There are a couple scenes in that movie that does a good job of showing how soft core porn is filmed. And it does a great job of showing how awkward it must be.
