So after about a year an half im finally wanting to get back in the gym. I'm looking for a good workout plan/guide to follow to get back motivated and also a good app to track what I eat. Thanks in advance

See this link.

Greyskull Linear and Candito Linear are the two I'd recommend first for someone who is basically starting from scratch, like you are.

I'm less of a food tracker, so I will defer to others there, but I feel pretty strongly on the linear programs.

I started using Fitness AI app about a month ago and I like it a lot. Has two versions of workouts for back/bis, chest/tris, shoulders, and legs that you rotate between each time you workout that area. I like it bc it took the guess work out of what I was going to do/how much weight/how many reps each day. I could've easily do that myself, but I wasn't.

It's a subscription ($8/mo) with only a 7 day free trial but I messaged the developer (easy to do in the app) and he gave me a free month to check it out.

Mike’s Macros for diet and the Anytime Fitness apps (one main one and one where you set your workouts) are working for me.

Don’t pay for exercise or diet plans. There is so much damn content on the internet by reputable people. And I mean actual strength coaches and dieticians. Stay away from instagram gym bunnies and gym bros. They’re just eye candy. Look up Johnny Pain and his GSLP program or Jim Wendler and his 5/3/1 book. Those are just two off the top of my head.
