When you go up the stairs by the courtyard with the witch looking broads with cleavers, there is a break in the fence on the right side. You can drop down there.
So when you turn that corner after leaving the room with the 2nd bell maiden, ignore the witches straight ahead in the courtyard and go up the stairs on the left that leads to the guy shooting lasers and more cleaver witches. Walk down the small set of steps on the right side and hug the fence line. You will see the break in the fence to drop down.

How should I proceed with leveling?
Level 93, use Ludwig's Blade +9 and Rifle almost exclusively
Vitality 30
Endurance 20
Strength 31
Skill 31
Bloodtinge 11
Arcane 20

That is almost exactly like my first build. I used Ludwig's Blade with it as well. The only difference is my endurance and Arcane are both 25ish and my bloodtinge is a little lower.
I stopped at lvl 100 and started a skill/bloodtinge build. I couldn't use any of the fun weapons.

He's in other Loran dungeons. You don't to fight him again to get to Yharnam, though you might be begging for him later.

Lower Loran is optional. I did it but it's really not worth it, unless you want to fight Abhorrent again and Paarl on steroids - LINK

I went out after work last night and got shiftfaced. Loaded up bloodborne at 3am and played.
I must've lost a grand total of at least 400k souls in Lower Loran
I kept trying to make it in that room with 2 spider maidens and 2 werewolves. I kept racking up 100k on the way and kept dying and I was so stubborn that I kept going until i did it. Cashed out with 200k

That room sucks even more when you realize there's nothing in there worth the pain of dealing with the enemies.
