Bad guy starts monologue of his entire evil plot instead of blowing good guy's brains all over the wall.
The dumb person-in-command who doesn't listen to good advice and is just there to be another obstacle for protagonist to overcome.
Every general in every movie. All are imbeciles and only want to nuke/carpet bomb/kill everything.

When a monster first appears in all its glory, everything stops as it spreads its arms and lets out a huge roar to announce its presence. Makes me want to throw tomatoes at the screen.
Vomiting. Directors seem to think taking time out to show a character vomiting his vomitus makes every stressful situation real.
The use of the word "shite", particularly on AMC and FX shows. It's like the approved curse word, and they overuse it to the point I wish they wouldn't use it at all.

This nearly led me to stop watching It's Always Sunny. They put such fricking emphasis on the word and it drives me crazy.

Good person has easy, clear as day chance to finally kill the bad guy as he's laying there with a knife in his leg or something, and the good guy just drops the gun and walks off so the bad guy can find them again
Detective is about to catch the bad guy, but his commanding officer tells him he must quit investigations because he doesn't believe him or is taking too long. Detective then has to go rogue

I'm tired of seeing good guys who refuse to simply kill a bad guy.
I hate when there's a brainstorming session and the people start finishing each other's thoughts. Cop shows do this all the time. Legends of Tomorrow does this too.
I understand why fight scenes can't be realistic. But really, if a guy is gonna be hit so hard that he literally flies through the air, or if he's knocked unconscious for a while...he's really hurt. At least show a black eye. It takes a lot of force to make someone fly through the air.

1) head injuries. One character smashes another character in the head with a frying pan or bat or wine bottle and knocks them out, but when the unconscious character regains consciousness, he suffers no concussion symptoms or brain bleeds or death (Supernatural is egregious. Love the show, but Sam and Dean shouldn't be able to remember their names with the number of blows to the brain they've had). This wouldn't be so bad if you didn't also have very minor fights, say, some dude shoves away a woman who's bugging him and it results in her tripping, hitting her head against a corner and instantly dying, resulting in the need to bury the body in the desert or something. So, are people's skulls made of iron or tissue paper? Cuz you can't have it both ways, Hollywood!

how ridiculously easy it is to knock someone out.
Sometimes a guy will say something like, "go to sleep!" a do like a half cocked swing that would barely carry a baseball from pitchers mound to first and the other guy is instantly knocked out cold.

A cop is involved in a critical incident and is back in action by the end of the day. No admin time off post-shooting, no fitness for duty evals/counseling, not even any paperwork.

I watched The Good Dinosaur for the first time the other night, and I noticed one that I hadn't really thought of as a cliche until that point.
During the climax, the hero will save someone right at the last second, then emerge from the water/rubble/etc. with saved person in hand. But, oh no, saved person isn't conscious. Are they dead? No, it's just a 10 second teaser to up the climax just a little bit more, and then they wake up and everything is ok.
