Check out the First-Time Abate. It's an administrative waiver that doesn't require a good "excuse" -- you may qualify if you have a good compliance history. It's discussed in IRM 20.1.1.3.6.1...

Thanks, please do. I'm referring to a document that doesn't typically need to be notarized but this particular company requires it. However, there is no place on the document/form for a notary to sign....
