He should go all the way and eliminate Veteran Hospitals completely. They deserve the best care they can get, which will always be private sector care.

What does "access" mean in the EO?
Don't get me wrong, this is great in theory. But if its along the same lines as the plan offered after the VA backlog nightmare, it was very ineffective. I ran a GI practice that was a large recipient of these backlogged vets, and it was an absolute clusterfrick.

I thought at one time a veteran being treated at the VA could request a second opinion and referral to a private physician?

Agreed, close down the VAs and just let us vets go where we want and our insurance pays the bill.

During the "wait time" fiasco, there was a program created that let vets go into the community for certain treatments and procedures that couldn't be done at the VA in an appropriate amount of time. Colonoscopies, for example.
My practice received 86 referrals in the first 2 days.
1) they had to be schedule and scoped within 30 days of the referral (not possible due to the practices other patients).
2) A precert had to be generated after the referral, and of course, no payment could be generated without a properly obtained precert, even though every patient was coming for a medical or preventative colonoscopy. Precert turnaround was 2 weeks+ at the time.
3) The VA repeatedly asked us to schedule patients while undergoing minor contract negotiations, then flat refused to reimburse for those patients or retro date the contract.
4) Payment only received for approximately 40% of the total procedures that were performed. It was so bad we had to drop the program after 60 days.
5) After the procedures, the vets could only f/u with the VA. Not the physicians who actually scoped them. The VA wouldn't pay for that visit, so they ended up back in the backlog, waiting for an appt to have their results interpreted by their VA PCP or GI if one was available.
They later cancelled the pilot project.
