As an MD, you should know that if there is a cancer or pathogen, and it is just "left alone", it will fester and grow to the point where it takes over the entire system. That is what we have in the Middle East--it can't just be ignored. just look at the state of Europe now.
When you are at the top (say, a business), you constantly have others trying to take you down--nature of the beast. You constantly have to protect your assets (customer base), and knock off enemies (the competition).
Same with world politics and control...there is a cost to stay the leader and in control.
