How ill does the POTUS have to be for the VP to be temporarily sworn in as President?

25th amendment sections 3 and 4. He decides or the VP and majority of cabinet decide.

He has to be unable to understand and make decisions.
If he goes on a ventilator then I think that will be the point.

If the president is not dead but just unable to complete his duties then the VP would assume those duties. He is only sworn in if the president passes away.
