I'm not all that tech savvy.
Are there any free apps or programs I can download that would allow me to create documents/forms?
This would just be basic documents with logos, line for name, address, and so on.
I have a Mac, but also have access to a PC.
What are some suggestions?

Who will be using the forms? Are they to be filled out electronically, or printed and written on?

Potential customers will be using the forms, hopefully.
Both, if that is possible, more than likely the majority will be filled out electronically though.

You can use google docs to create forms, then the inputs to those forms will be stored in a spreadsheet. I haven't done too much with it, so I'm not sure about adding logos, etc.

Also, if you are filling out forms yourself, you can use a program called Tap Forms for iOS. Pretty nice UI, and can export as a CSV.

Adobe Form Center/Creator is pretty decent, but the templates suck so you have to make your own.
