Hint: you can do massive damage and cause much butthurt with the chikage and 40 bloodtinge.

Apparently you can get into iosefka's clinic through a jump glitch. Did it last night with a new character

I've abandoned bloodtinge on this build. It's at 9 with arcane at 20 and everything else at least 25. I think once I get arcane to 25, I'm going to max out skill and strength. I had no idea what I was doing when I started leveling so I've had to adapt my build to my early game decisions.

How do you raise your left hand attack? Strength is only increasing my right hand attack .

I thought so, but for some reason I don't remember it raising. Maybe you have to do it multiple times.

I don't use it as often as I should but I still use it quite often. My bloodtinge is only 9 so I use my gun strictly to parry unless I'm using my cannon. Some people level bloodtinge to 40 or 50 if they want to use guns as more of a damage dealer. It's up to you on how you intend to use the firearms.
ETA: To compensate for my low bloodtinge, I use bone ash marrow with my cannon when I want to deal damage long range.

Chikage transformed state has S scaling with bloodtinge

You can also do quite a bit of damage in pvp with an upgraded Evelyn or Repeating Pistol.

I killed The One Reborn this weekend and then found out I completely missed Cainhurst and Iosefka's Clinic. Hopefully I can backtrack without messing anything up. This is the third thing I've screwed up this playthrough. The first two being Eileen's quest and then killing the Oedon Dweller. I pretty much botched the whole recruitment side quest. I haven't been able to send anyone to the Cathedral or Clinic.

Definitely go to Iosfka's Clinic after beating Rom. You get there through the Forbidden Woods. The path behind an NPC and dogs in cages.

Does anyone recommend buying either Ludwig's Rifle or the Flamesprayer?

There's really nothing special about the rifle and the flamesprayer is useless unless you have points in arcane.
