Help me upgrade my computer

So I got this computer a pretty long time ago and it is starting to become sluggish. I run dual monitors and like to play game or browse internet on one monitor and stream watchespn or twitch.tv or something of the sort on the other one.
My specs are (don't laugh)\
Intel i5 750 @ 2.67 GHz
8.00GB RAM
Geforce GT 220
500 GB HDD
Which should I upgrade first? I'm thinking video card and move to SSD, but I basically don't know anything.

Before anything I would format the disk, wipe it clean and do clean installs. Only install what you need

I've done that, it just seems like the hardware can't keep up anymore.

That's the same proc I have, I put an SSD in mine and I'm happy. If you're gaming, that video card sucks.

If you're talking about general sluggishness, after you have msconfig-Ed the thing to death, an SSd upgrade will likely be best.
If you're talking about gaming performance, I'm admittedly out of my element, but your video card should be upgraded before the cpu. Your cpu is old, but it shouldn't be giving you trouble unless you're doing some sort of video converting or otherwise heavy-lifting in the background that you've failed to mention. I can't give any recs as to which video card you should move to, but look at ILikeLSUToo's gaming PC thread and read it for the GPU- ignore the cpu and motherboard as you're already set there.
Someone smarter will likely tell me I'm wrong. Trust them.
