It is flooded timber for ducks, but only holds water for 2 months out of the year.

Don't call Jennifer. There is a nationwide permit that shouldn't require notification to the Corps for this. I think it's NWP 27. Go to the USACE Vicksburg website, regulatory, permits, nationwide permit summary, and look for the one that fits your project.

Is the structure within 1500 ft of the levee and visible from levee?

And I would NOT cal corps if possible.

Yes, there is already a culvert there. It is not within a mile of any substantial levee.
