Sister has small poodle that is having an allergic reaction to something. Has no fleas but seems miserable. Any thoughts or ideas?

1 mg of benadryl per pound of body weight 2-3 times daily until the symptoms subside. I'd also talk to the vet about getting monthly allergy shots as these help a lot. Other than that play with the diet and change food. My bulldog is allergic to beef and corn, so the dog food we get is salmon/chicken/lamb and peas/rice for her. A lot of things you can do, but the benadryl to help relieve some of the symptoms in the mean time would be my first approach.

Apoquel (sp) is the BEST for itchy allergy dogs.
I've also used Zyrtec

Generic Claritin. Also, Benedryl if they're really agitated. They can have a mg/LB per my vet. they're just going to be lethargic for a while.

I'm assuming miserable and allergies = lots of scratching? If so Benadryl as recommended above is a good place to start and you can also put a little olive oil on their kibble as that can help with skin issues. If that doesn't help or you used the phrase "allergic reaction" because there's a rash, you'll probably need to go to a vet.

Tell your sister to bring it to the pound and trade it in for a real dog.

normal benadryl is fine.

My bulldog used to take this, but she started getting tumors that could (though not 100%) have been the result of the way Apoquel blocks allergens and effects the dog's immune system. She's moved to cortisone shots to ease some of the itchiness she gets from non-dietary allergies. I'm not saying Apoquel is bad, it worked great, but it can have a negative impact on certain dogs immune systems.

Oh. Well then I guess I may pause in going back to get him back on it. I just haven't had time.
The Zyrtec isn't as good but definitely better than nothing. Plus, we can share a bottle

Benadryl is my remedy. Steriods only when it gets bad.

My dog is on Apoquel 1.5-2 pills a day. I also give him a Claritin in the morning and two Benadryl at night, he's 70 lbs. I just started him on hill science derm defense foo on Saturday. And also takes a steroid pill every 2/3 days. No shots yet. Waiting to see if the food helps. I'd love any input.

always used Benadryl, depending on the size dog you might want to break it in half

We did 1.5 pills a day for about 9 months (IIRC) with her before she started to get growths. We weren't exactly sure what was causing the growths, but had them checked out and they ended up being benign. Was decided to go to a steroid shot as needed (45-60 days depending on the season) and benadryl nightly. The growths that were removed did not come back. Though she did get one that was tested positive when it was removed about 6 months later. Have not had any since so hopefully it was isolated though the initial labs were thought to be mast cell which untreated would have given her about 2-4 months, we're on month 14 now though so hopefully it was a mis-diagnosis. That's honestly the only input I have, I was told the initial tumors were most likely caused by a negative effect on the immune system due to the way apoquel helps block the allergens. When the tumor tested positive and appeared to be mast cell I assumed the other tumors coming back may have just been coincidence, but with no new growths in over a year I'm beginning to lean back to the original diagnosis of the drug causing the tumors.
If you're unfamiliar with mast cell it is a very aggressive cancer that causes tumors, high histamine levels, and dietary issues. Which were all signs we saw. Due to the aggression of the cancer though it is uncommon for an untreated dog to last three months and rare for it to make it past month four. With chemotherapy nine months is common, and a year and beyond is rare. Untreated living for 14 months with no new tumors isn't rare it's unheard of.

Medicated shampoos and perhaps low dose of allergy meds.
