Depressing story
My dad used to skydive alot of of Gonzales, jumpmaster and all that jazz. One jump he goes up with a rather novice jumper. When they exit, they are doing some kind of formation, at that tone my dad sees that the other jumper doesn't have her parachute properly on. He attempts to signal this to the other jumper, which she doesn't recognize. He reached the point where he had to pull the chute and when she went to pull she came out of the chute. Died on impact. My dad never jumped again.

Update in OP after the instructor was ID'd.
They also have a photo of the "Skydive N'awlins" business at Slidell Municipal Airport, so it could have originated from there.

She was a surgical tech at Our Lady of the Lake, I knew a number of people that knew her.

Back in my day we would go over each others harness and rigging before getting in the plane, so was very shocked when this happened. Some speculated that she had the lower thigh straps hooked up when she got in the plane, but un-hooked them in the plane to be more comfortable and forgot to hook up again. I had already quit skydiving and flying for several years when a jump plane crashed on take off killing several people I knew, that sealed my decision to find some new things to do with my time.

I had a friend who died in a skydiving accident. He had almost 800 jumps and had been in a parachute regiment of the British Army. He was very experienced, but he fricked up and died.
Also, I just read an news article reporting the death of a Navy SEAL this afternoon during a Leap Frogs demonstration in New Jersey. He had some type of parachute malfunction and ended up in the Hudson River.

I believe I remember this. Happened somewhere in the mid to late 90's?

Yes definitely a while back, I can't even remember what grade I was in. Just remember him telling me what happened and then never jumping again.
