Question for OT Doctors and Patients

Do you guys think that the Facilities costs of surgery are inflated?

I can understand the high cost for Surgeons and Anesthesiologist but it seems that facilities cost are a little high.

I say this after getting a pre-insurance quote of $2,100 facilities cost for a 45 minute out-patient surgery.

What do you think?

No, not at all.

A Tylenol that costs $85 dollars at a hospital is worth it. And a hospital CEO who makes $1.5 million at a not-for-profit is a bargain.

The services billed vs. allowed billable vs. billed to patient vs. received as reimbursement vs. recouped by insurance company after the fact game and its inherent problems coupled with the large under funded or unfunded population have created a shocking and disproportionate inflation of services billed.

Not at all. A pair of crutches on Amazon are the same as at the hospital, around $900.00.

Yes, it is more expensive than it should be because you aren't just paying for your hospital visit. You are also paying for all of the POS that don't pay a dime, as well.

Yea, they are high. It's accounting for alot of labor, products, and administration. But yea, even the allowables are really high.

At a hospital or ASC? That sounds very high, but I believe it. A colon at an ambulatory ASC will run $600-$800 for a facility fee. Upwards of $1000 if done at the hospital.

It's sickening and an embarrassment.

Fun one I recently learned- it would be a federal offense for us to discuss our fee schedules with each other. It would be looked upon and treated as price-fixing.

Federal offense??

Breach of payer contract, yes. But I can't think of how it would be criminal.

At an ASC that's not horrendous. Our facility fee is not that high but it ain't much lower. You can't charge individually for everything you use at a surgery center. So for a case I do, for example, the tourniquet, the foam padding, the clean up crew, the cost to process/sterilize the instrumentation, the hardware costs, the scrub tech, the circulating nurse, any drugs you get in PACU, etc all come out of he facility fee. The hospital rolls some of that into their facility fee, but can itemize and charge for everything otherwise.

But hey, let's make it harder for physicians to practice outside of the hospital because why do anything that has been shown to be cheaper and often times more efficient?

I'm told it violates anti trust law for some reason I don't fully understand, take the statement as fast, and don't discuss fee schedules with other physicians.

Wait until we get the bill for all the flooding thru insurance premiums.

I had a Total Knee Replacement last Nov. 9th.The Surgeon charged,I believe,$12,500.That was a fraction of the total Costs after 3 days in the Hospital.

Heck,the Anestisiologist(sp) charged a couple of grand.
