Other than the headache and residual high BP she had no symptoms by the time she got to the ER.

Any reason they didn't do a CT scan? Has she had carotid dopplers, echo, (stroke work up) with her prior TIA?

I wouldn't let her BP run too low. Want to keep the cerebral perfusion pressure adequate.

Someone mentioned PRES, but that shouldn't cause confusion, but more ataxia/dizziness.

She had an MRI and I believe ultrasound after the TIA. It was three or four years ago. Maybe a little longer. Nothing unusual found at that time.

While her blood sugar may have been normal by the time she was seen, it is possible that she had some transient hypoglycemia.
When you say mental confusion, how did it manifest?

She got up this morning was making her bed and felt an odd feeling come over her. She said for a few moments she was confused about the steps involved in making the bed.

Has she been to a hairdresser recently? Leaning her head back getting her hair washed can cause CVA's as can turning her head to look backwards while backing her car up.

Head CT is standard with mental status change, especially with geriatrics with a previous TIA.

Very odd is was not done.

It could be a UTI. Sometimes confusion, dizziness, and headache are the only symptoms the elderly present with. Did they take a urine sample?

I'm not a physician. Get a second opinion if you still concerned and not satisfied with the ER doc's decision.

That's an ER doc with a set of brass balls to not CT a 70+ year old woman given those symptoms.

Was this an ER or Free standing ER? Our place had a busy fricking week pre holiday. Hospital was full.

Re: neck hyperextension--Wouldn't that be a vertebral artery CVA in the cerebellum distribution, causing ataxia, maybe nystagmus, but no mental status changes?!!

My mother is in that same age range, and has something similar every few months. After an extensive neuro workup, she was diagnosed with "aborted migraines". Despite the name, she gets a headache, gets very confused for a short while, then goes to sleep, and wakes up very angry. Taking Maxalt at the first signs lessens the severity and duration of the symptoms significantly.

Update: another episode with loss of consciousness. Back in the ER she can talk and move her limbs and knows where she is. Waiting on more tests. They will probably send her to Shreveport

Best of luck. I'd be pretty adamant about a head CT and EKG at this point.
