On call Thursday night I intubated a patient who was status post a tracheocutaneous fistula closure from earlier in the day. She had previous head and neck cancer that required chemotherapy and radiation for treatment. The kicker? She had no epiglottis after the radiation and had a significant amount of laryngeal edema as well. Want more? The reason for the bring back was a SIGNIFICANT amount of subcutaneous emphysema that basically inflated the entire left side of her face like an overinflated balloon. I guess air was being entrained subcutaneously after incomplete tracheal closure. To top it of she had just had PO fluids so we had to do a rapid sequence induction. Anatomy was wack but the tube found the trachea. Butt puckering for sure.

Mostly routine I gather, but my coworker sure as hell had an awful week after his groin hernia surgery because his intubation didn't go "routine." Hernia repair was least of his worries. Couldn't eat or talk much of shite for that week ... It sucked and he was miserable. Hope your patient is in the 98% "routine."
