I had the same thing a month ago. me and a semi-pro friend went at it. he noticed that the original I had in the wall had wires crossed but we did not photograph before we took it out.
after messing up for two hours, I paid a certified electrician $85 and it was done right in 20 minutes.
side notes
the first time I got a replacement part, I got it home to find it was NOT a 20 although it was in the 20 bin . went back after could not get it to work.
this did not have a part in not getting it to work, as far as I can tell after the fact.
a 15 should have worked but not lasted as long.

Could be reverse polarity. I've worked on them before and that's been the issue.
Just verify you have identified the load vs the line side.

A bad ground will cause a GFCI to do this. I see this post is two pages long and am sure this has been answered by now. Cheerio.

Perhaps it is working exactly as intended and there is a ground fault, kooky hunh
I would check that out or you could go buy a few more GFCI outlets and see if that resolves the problem.
