George Jefferson was not originally play by Sherman Hemsley on All in the Family.
Does Dr. Who count?
Sonja in Mortal Kombat.
Ensign Pulver in Mr. Roberts was Jack Lemon, not so much in the sequel. Captain was James Cagney in the original and Burl Ives in the sequel. That's just a little bit different.
Superboy during the 90s series switched out actors in the lead like Spartacus.

I don't know that I would actually count that one as it was explained that Neo recognized a part of her, and that she had to change her "shell" due to certain circumstances. Also the original actress died before being able to make the third installment, but the filmmakers were able to write it in the script to explain the change. I think what the OP was saying is when a movie presents a certain character and then in the next movie-POOF! new actor, no explanation.

Morty Seinfeld and Ross' lesbian ex on friends were played by different actors in the pilot. Not sure if that counts.
