I think he's referring to the Jaime/Qyburn scene. Jaime wanted to be conscious during the procedure so that he could make sure Qyburn wouldn't amputate the whole forearm.

It's not just that scene. Everyone who is offered it refuses. There might be a couple of exceptions, but I don't remember them.

They did. Some do, some don't. Tywin in Season 3 also tells them to give it to Joffrey when he tries to refuse being sent to bed after they found out about the Red Wedding.

I remember reading the Hodor thread last year, and you were getting depressed over Tyrion and Sansa's wedding, saying it was the worst and saddest wedding ever. Had to laugh knowing what wedding was up next.
