I'm talking about REAL games (not Pre season or practice)

All I can think of are the Yankees and Alabama football

USC wore some weird metallic helmet within the last few years. Their jersey has also evolved slightly, but for all intents and purposes it's been the same

Bama also had a period when they had white helmets along with their crimson helmets. Every team has changed their look to some degree.
