I use adfree time on a recommendation from other users on this site and subscribe to the MLB.tv package.
$1.99 per month for adfree and I paid $100 for the mlb package at the beginning of the year. I then use the app on my Fire TV or Xbox360 and all games are region unlocked.

FYI - video still doesn't work on my phone or tablets for MLB at bat, I assume because of GPS on those devices.

You don't have to pay. I have the Texas package through MLB.tv and use the free Hola VPN chrome extension and it works great. Every month or two I have to clear my cookies, but other than that it works great.
