Just beat Rom on my 3rd try.

Kill the small spiders, then cheese him. When he goes to teleport, do a charge attack with heavy weapon, should stop his teleporting, allowing you to keep killing him.

I ignored them completely when I fought him in the main game. In the chalice dungeon I killed the first batch and then ignored the rest. He was much harder in the chalice dungeon for me because the area wasn't nearly as open.

I don't think you can kill her until you get into the clinic. When you attack the door I think you just scare her away. If you have access to the forbidden woods look up the path to take to get into the clinic and see if she's alive.

I went up in the cathedral ward but at the top of the tower was a door I couldn't open.

You get the key in Yahar'gul, the Unseen Village which can be accessed after defeating Rom. The key is in a locked cage that you drop into from the level above. It's an annoying area because of all the bell maidens so make sure to kill them in each area first.

For anyone going for the platinum trophy make sure you grab the 2 red jelly in the lecture hall before you go to NG+. That way you have more options to get the other 2 required for the Great Pthumeru Ihyll Chalice.

After about 30 deaths to the Pthumerian Descendant and the completely fricked up inconsistent parry window I'm about to smash my PS4 into a million pieces. I've never had this much trouble with a boss.

She has Mayor McCheese eyes on the inside of her head. I saw on Reddit, someone took a screenshot of the doll's eyes and she has a second pair of eyes inside her eyes.
By far the hardest boss I've fought so far.

Did you beat him in the Great Pthumeru Ihyll Chalice? If so what did you do? At first I was about to get him down with parries but it because so inconsistent that I wasn't getting any in some fights.
ETA: frickING FINALLY!!!!!!!!!! I had to resort to cheesing the shite out of him but I don't care. frick Mr. my Shotels can fricking fly through brick fricking pillars and rape the shite out of you. By far the hardest boss for me in this game bar none. He's so fricking fast that if he doesn't kill me with one of his combos he does it 1 second later before I can get away and heal.
