Just bought PS4

Haven't had a console since the PS3 back in 2012 when it crapped out on me during my final days of law school. Out of the loop on games now.

anything from COD, GTA, First person shooters, and all EA sports. I like the open world games. I've heard alot about Fallout 4 and Assassins Creed. Ive never seen either one played. Same for all the Drake's Uncharted games and the Last of us. Heard good things but never played.

So the first three should be Fallout 4, Grand Theft Auto 5, and either Black Ops 3 or Battlefield 4. BLOPS3 came out recently and BF4 is over 2 years old.
The Uncharted games and Last of Us got an HD remaster for the PS4, so those would be good options as well.

Based on what you said you like:
GTA V first since you like GTA games. Awesome and really deep game. Online is robust if that's your thing (I hate online so I couldn't speak to it).
You said you like EA Sports games. I would pick up NBA 2K16. It's not EA, but it's probably the best sports game on the market besides The Show. I don't have a PS4 yet so I have no idea how The Show is on this gen. It was pretty great on last gen. NBA is a huge jump from last gen to the current gen. I used to love Madden games, but I find they just don't hold my attention for very long any more. Very pretty game and gameplay is not bad. Maybe I'm just getting older.
I would highly recommend Witcher 3. That's probably the best game I played in the last couple of years and took a solid 120 hours to get through. I didn't think this was my type of game but it was incredible. Don't wait too long to try this one!
Some other games I really enjoyed:
Assassin's Creed Black Flag was really cool if you like that series. Unity (post launch patches) and Syndicate are also pretty good games in the series.
The last two Tomb Raider games were really good. The most recent one was the best, but I would recommend playing both and in order. You could also skip the first one and not miss much, which may be a good option since you have a backlog of games.
Fallout 4 seems to have mixed reviews. I haven't played it but would recommend reading some reviews to make your decision.
I enjoyed Batman Arkham Knight and just started a second play through. However this game also had mixed reviews and I may like it more than most.
The Mad Max game was pretty awesome.
Shadow of Mordor was also a pretty fun game.
There's a couple of games from last gen that you may have missed that were remastered for current gen. They didn't get the most attention but were very fun for me and got multiple playthroughs. Sleeping Dogs was one of the best games I've ever played. Graphics aren't great (not bad either) but it's a really good game.
I also loved Saints Row 4. I'm not a huge Saints Row fan either. 3 was the first one I liked in the series. The addition of super powers in 4 made this game one of my favorites of the last few years. A very stupid game...but a very fun game.
If I was you, I'd start with GTA V and a sports game. You can jump back and forth to the sports game for spurts. After GTA I would go straight into Witcher and immerse yourself into that world.

I feel like Bloodborne may not be the best game to start out with for someone who hasn't owned a console in awhile.

If he has any interest in Souls games it might be a good place to start. More accessible from what I hear.

compile a list of games you may think you want.. spend an hour or so on LINK and search for each game and watch some footage of it. You can see the game in action + ask the streamer their thoughts on it.
