You still get things like caprese on crostini hors d'oeuvres and the like. You can plug the definition, but one that doesn't have a ton of qualifiers probably needs to be built from the ground up.

bread, meat, condiments, with the bread used as a delivery vehicle.

Every culture has a sandwich of some sort or fashion. Food on the go is a requirement of human life and sandwiches have long been the preferred and technologically possible means/method.

Sure, but even the strictest of sandwich definitions (meat between two slices of bread) would engulf hamburgers, so saying that hamburgers aren't sandwiches would require the definition to be either tautological(sandwiches are things we call sandwiches) or absurdly arbitrary (sandwiches are meats between two slices of bread except for when we call meat between two slices of bread something other than a sandwich).

Frankly, I find hotdogs fit far more easily under the "sandwich" heading than I do "open faced sandwiches" which seem to defy the entire purpose of the sandwich to begin with.

My definition of bread always required some kind of rising/leavening.

That strikes me as an awful slender reed. Also, if I put chili in a bread bowl and put the hollowed out piece resting on top, that makes it fit the definition. In the same way, if I have a sausage kolache, that isn't a sandwich, but if I split the bread it is. Strikes me as an arbitrary distinction without a real difference.

P.S.
lots of unleavened breads out there used for sandwiches yet still breads. If specifically yeast leavening required for bread, then we get into: is cornbread a bread?
