Yeah I think it may have been a friend of their's that was starting a magazine called Metallica and they used the name before he could.

They were originally called Mookie Blalock, but 10 is a reference to the number of surfing titles won by Kelly Slater.
Another one - The name of Alice in Chains' current singer William Duvall's former band is Comes with the Fall. They are named after a scene from the movie Rosemary's Baby.

Grateful Dead were the Warlocks. Phil Lesh said he had seen a record in a store by The Warlocks and they didn't want to steal the name.
They were sitting around an apartment in Palo Alto trying to come up with a name and Jerry opens a dictionary. He looks down and sees "Grateful Dead" on the page and that was it.

In 9th grade a group of kids would play music at parties and wanted to sound hard and badass. After taking a freshman English class and learning about the cool word for demon, Incubus was born.

I've read this story for Flight of the Conchords on their website before. Not sure if there is a bit of truth or all a joke (leaning towards joke)
