I had same thing a while back. Turned out to be a prostate infection. The pain radiates down to the nuts.
Doc says he sees it a lot in truck divers. Something about the constant jiggling while driving. I'm not a truck driver but at that time I was driving quite a bit in a hard riding pickup.
Long story short, I got an antibiotic prescription and a softer ride and no problems since.

So my nut pain has subsided as the week as gone on. Yesterday wasn't as bad as Monday, today isn't as bad as yesterday. There's still some discomfort there, but it's not as consisent as it was. I think whatever it was is going away.

This happened to a fraternity brother of mine. Turned out he lifted too much (was helping people move in) for too long without taking a leak. Turns out if you do that you can end up peeing into your own nut sack.

I thought I had a hernia once because of discomfort and a lump on my left nut
Turned out to be a spermatocele

No idea. It's just what I was told. Apparently it his ball sack swelled up to the size of an orange before he went to hospital.

A hydrocele isn't painful, it's just fluid swelling. Torsion is an altogether different scary animal. One of my fraternity brothers had that.
