OMG! I think if my heart beats any faster, I may have a heart attack right here and now!
Not standing is, of course, included in the "poor manners" about which I speak.

What about in a business setting? I'd personally avoid doing this. The consequences of possibly getting it wrong outweighs the potential benefits of getting right in my opinion.

Where are people like you when I'm out to dinner with my wife? I need a break from what seems to be the norm now. The quality of the restaurant shouldn't matter... "Manners are Manners"
I get so frustrated with the lack of manners these days. It absolutely infuriates me when I see men not holding doors for ladies or even worse, walking through the door in front of them. No more chair pulling, no more standing when they return from the restroom. I don't know what has happened to manners.
My 17 year old daughter has grown to expect good manners from boys she dates and she knows that any boy she brings home better display good manners or I'll throw his arse out. I started teaching my son proper manners when he was about 7 or 8 and now holding the door and pulling the chair for his mother or sister are second nature to him. We don't always stand when they return to the table because we usually end up in a booth when we eat out as a family.
OK, enough ranting.
To anwser the original question, it bothers me when people order steak well done or they don't go out on a limb and try new things. Especially my wife's sister... I took her and her husband to Ruth's Chris a few months ago as a "thank you" and she ordered a well done Filet and onion rings. The waiter nearly shite his pants. He looked at me and I just shrugged my shoulders. I didn't know you could cook a filet well done. The whole thing was a mistake; I should've taken them to Chili's.
