Could also have been a ruptured ulcer. Know someone who had that happen and puked blood everywhere. Luckily in an urgent care clinic at the time and they got an ambulance there and she's ok.

I've told the rest of that story here before. Because of where the apartment was located and some disputes going on with jurisdictions, Pops had a hell of a time getting someone to come collect the body. A few hours and several calls later he told someone on the phone that he was just going to tie a rope to my uncle's feet and drag him down the stairs (upstairs apartment). Police were on scene within ten minutes and had my Pops in cuffs. The body was collected and Pops didn't go to jail.

I’ve triaged a man who had a heart attack in SF but boarded the plane after eating aspirin. Gutted it out with nitro the whole flight. Horrendous EKG.
Also triaged a guy with a splint from his arse to his toes. It was put on in Nigeria. After he was shot in the thigh with an AK. He lied said it was his knee to get on the plane. Flew 16 hours with a femur in about 12 pieces.
He was smart. Nigeria is no place for trauma surgery. The SF guy was an idiot. No shortage of cardiologists in SF.

I am reading The Hot Zone right now. It’s a non-fiction book about infectious diseases.
The book opens with taking us through a Frenchman getting sick in Kenya, and getting on the commercial plane to travel to the good hospital. On the plane, he pukes up so much blood he fills an air sickness bag.
Once he gets to the hospital, he has such explosive blood diarrhea in the emergency room waiting room he shits out the inner lining of his intestines.
This book is not making public transportation seem appealing.

Sounds like his pulmonary artery ruptured…or possibly an aortic aneurysm rupture.
Had a pt with a tumor sitting on his pulmonary artery that eventually eroded and it ruptured the vessel. It was like a blood bath. Just like what was described, blood gushing out his mouth and nose. Poor guy died before we could get him intubated.

Almost happened to me. Luckily I was at home, and not on a plane.
I was taking too much ibuprofen due to aches and fever while still going to work and it turned into a scene from Carrie.
30 minutes earlier and I would have covered the windshield inside of my truck with blood and passed out on the side of I-10.
Instead I get home and go to the bathroom, where suddenly I turned into a fountain of blood. Barely made it to the ER thanks to my wife, where they stopped the hemorrhaging and gave me a blood transfusion.
Turns out I had Stage 4 esophageal erosion, with no pain, or any warning signs, and the Ibuprofen had caused massive bleeding that filled my stomach with blood.
I had suffered from acid reflux years before, but it had been gone for a while. I thought.

He died very badly. But not before coughing blood into the moth of the doctor who was treating him. Who promptly got sick a week later and was confused why he got sick lol.
The book just had an odd way of phrasing it lol. To paraphrase very badly “… Jim started to feel odd. He had back pain. But he was getting old. And 30 years was old! Back pain is what 30 year olds got! But then he started feeling worse. And worse and worse and went into a coma.”
